Relação do débito calórico com a perda de massa muscular medida por ultrassonografia em pacientes críticos

Resumo

Objetivo: Estabelecer a associação entre o débito calórico e a perda de massa muscular medida por ultrassonografia em pacientes adultos internados em unidade de terapia intensiva (UTI). Método: Estudo observacional de coorte prospectivo, incluindo 101 pacientes, com permanência não inferior a 7 dias na UTI. Com um processo de seleção de amostras consecutivas. Um total de 3 medidas da espessura do quadríceps foram realizadas por ultrassonografia nos dias 1, 3, 5 e 7. Essa espessura muscular foi medida com um transdutor linear de 10 MHz, por um médico treinado. Dois locais de medição foram avaliados: no ponto médio ou dois terços do comprimento entre a espinha ilíaca ântero-superior e a borda superior da patela. As variáveis ​​avaliadas foram: espessura muscular do quadríceps, percentual diário de cumprimento das metas calóricas e proteicas. PRINCIPAIS RESULTADOS: A espessura do quadríceps femoral diminuiu em até 20% durante a primeira semana após a admissão na UTI. Não foi encontrada correlação entre perda muscular e débito calórico ou proteico durante a permanência na UTI. Conclusões: A avaliação ultrassonográfica da espessura do quadríceps pode ser considerada uma ferramenta para avaliar a perda muscular em pacientes críticos.
