Nutritional aspects in patients with chylothorax in conservative treatment on enteral diet.

Introduction: Chylothorax or chylous pleural effusion is a rare condition characterized by the accumulation of chyle, also known as lymph, in the pleural space. It occurs mainly due to surgical procedures or changes in the lymphatic duct, such as obstruction caused by neoplasms. Goal: Analyze the nutritional evolution of children hospitalized with chylothorax using an enteral diet rich in medium-chain triglycerides (MCT) and free of long-chain triglycerides (LCT). Material and methods: This is a retrospective and longitudinal study, carried out by consulting medical records located in the Medical Archive and Statistics Sector (SAME) of a co-participating philanthropic institution for analysis. Results: Of the total sample (n=18), 100% surpassed chylothorax only with the enteral diet rich in MCT and free of LCT, without the need for the use of medication (octreotide), surgical intervention and/or total parenteral nutrition. As for the influence of nutritional status, the parameter of weight for age at admission was inversely proportional (p=0.025) to the length of stay in days. Conclusion: The nutritional status at admission can be determinant for the development of chylothorax and length of hospital stay. The MCT-rich and LCT-free enteral diet is an effective and non-harmful treatment option for children. Therefore, it is extremely important to assess and monitor the nutritional status of these patients.
